Carry a Collaborative Care caseload for patients in primary care. Reach and engage patients, deliver brief evidence-based interventions, and keep the registry current alongside the consulting psychiatrist and the patient's PCP.
- NY-licensed clinician (LMSW, LCSW, LMHC, or LMFT)
- Comfortable with telephonic outreach and engagement
- Tracks PHQ-9 / GAD-7 and follow-up in a shared registry
- Bilingual (Spanish or an Asian language) a plus
A senior, independently licensed clinician who both carries a Collaborative Care caseload and provides clinical supervision to associate-level care managers.
- NY independent clinical license (LCSW, LPC, or LMFT)
- Eligible to provide clinical supervision in New York
- Experience with measurement-based, collaborative care
- Registry and psychiatric-consultant collaboration
