NY Licensed Therapist / Behavioral Care Manager
A full-time, NY-licensed clinician carrying a Collaborative Care caseload in primary care. Reach and engage patients, deliver brief evidence-based therapy, and keep the registry current alongside the consulting psychiatrist and each patient's PCP.
About the role
Integral Health delivers the Collaborative Care Model inside primary care practices. As a Behavioral Care Manager you are the engine of that model: the clinician who reaches patients, supports them between visits, and makes sure no one is treated and forgotten.
This is a full-time, remote, 1099 role serving patients across New York.
What you'll own
- Carry a Collaborative Care caseload. Reach and engage patients in primary care, and support them between physician visits.
- Deliver brief, evidence-based care. Provide short-term, measurement-based therapy and behavioral interventions.
- Keep the registry current. Track PHQ-9 and GAD-7 and follow-up alongside the consulting psychiatrist and the patient's PCP.
What we're looking for
- NY clinical license (LMSW, LCSW, LMHC, LPC, or LMFT)
- Comfortable with telephonic outreach and engagement
- Tracks PHQ-9 / GAD-7 and follow-up in a shared registry
- Therapy plus care-management experience a plus
