Therapist / Behavioral Care Manager (LMSW, LCSW, LMHC, LMFT) – 1099, Part-Time, Remote – NY
Provide individual behavioral health therapy within the Collaborative Care Model (CoCM), working virtually alongside primary care providers and psychiatric consultants. Manage an assigned caseload, deliver brief evidence-based treatment, monitor progress, and coordinate care as part of an integrated clinical team.

Provide individual behavioral health therapy to patients within the Collaborative Care Model (CoCM), working virtually alongside primary care providers and psychiatric consultants.
Manage an assigned patient caseload, deliver brief evidence-based treatment, monitor patient progress, and coordinate care as part of an integrated clinical team. This is a part-time, remote, 1099 role serving patients across New York.
What you'll own
- Carry a Collaborative Care caseload. Reach and engage patients in primary care, and support them between physician visits.
- Deliver brief, evidence-based care. Provide short-term, measurement-based therapy and behavioral interventions.
- Keep the registry current. Track PHQ-9 and GAD-7 and follow up alongside the consulting psychiatrist and the patient's PCP.
What we're looking for
- Active NY license: LMSW, LCSW, LMHC, LMFT, or equivalent
- 2+ years of experience providing behavioral health treatment
- Experience providing individual therapy
- Comfortable with telehealth, EHRs, and measurement-based care
- Able to commit to a minimum of 15 hours per week
Nice to have
- Adolescent behavioral health experience
- Bilingual Spanish
