Adolescent Licensed Therapist / Behavioral Care Manager
A part-time, NY-licensed clinician providing therapy and care management for adolescents in primary care through Collaborative Care. Engage patients and families, deliver brief evidence-based interventions, and track outcomes in a shared registry.
About the role
This role brings Collaborative Care to adolescents inside primary care. You will combine brief therapy with care management, working closely with families, the consulting psychiatrist, and each patient's PCP.
This is a part-time, remote, 1099 role serving patients across New York.
What you'll own
- Support adolescents and families. Engage adolescent patients and their families, and support them between visits.
- Deliver therapy and care management. Provide brief, evidence-based interventions and coordinate the plan across the care team.
- Track outcomes. Keep PHQ-9, GAD-7, and follow-up current in a shared registry.
What we're looking for
- NY clinical license (LMSW, LCSW, LMHC, LPC, or LMFT)
- Adolescent behavioral health experience
- Comfortable with telephonic outreach and engagement
- Measurement-based, collaborative care
